HCBS Self-Assessment Information Due March 31, 2020

As part of the Department of Developmental Service’s (DDS) efforts to meet requirements of the Home and Community-Based Services Final Rule, all eligible day, employment and residential service providers are required to complete a self-assessment to measure their current level of compliance with the HCBS Final Rule. The required self-assessment is available through an online portal.

DDS has identified that all providers with the following service codes must complete the HCBS self-assessment.

  • Residential: 096, 113, 904, 905, 915, 910, 920

  • Day Programs: 028, 055, 063, 475, 505, 510, 515, 855

  • Employment Programs: 950, 954

    All identified service providers must complete a self-assessment by

    March 31st, 2020

  • Trainings:

    • DDS is hosting five statewide trainings with both in person and webinar options during the week of February 10th, 2020. Date, location and registration information is available on the attached flyer. To attend a training in-person, register using the link on the flyer. Participation in one of these trainings is strongly recommended by DDS.

    • Technical assistance on navigating the self-assessment is available through the online portal. This includes frequently asked questions, training videos and a help desk ticket system to ask questions of the regional center or online portal IT staff.

    • NBRC will host two live screenings of the following DDS Provider Self-Assessment trainings:

                          – Thursday, February 13th, 12:30 pm – 4:30 pm, NBRC Santa Rosa Office

                          – Friday, February 14th, 9 am – 1 pm, NBRC Napa Office

    Action is needed by your agency! If a representative from your agency has not already received an email with logon credentials, please go to the online portal using the link below, and click on “Need an account”. This will allow your agency to register and access the required self-assessment.
