Alternative Service Delivery Monthly Report Information

Hello Non-Residential Service Providers,

This communications pertains to non-residential vendors that are providing Alternative Services. This specifically excludes vendors under the following service codes: 096, 109, 113, 896, 900, 902, 904, 905, 910, 915, and 920.

If you have been providing Alternative Services and have billed your monthly averages for the months of September, October and November, this information is for you. If you have continued to provide traditional services only, this does not apply to you.

Below is the latest guidance from the Department of Developmental Services (DDS) on the Alternative Service Delivery Monthly Report and a sample of the monthly report.
