There may be times when the planning team determines there is a need for regional center funding to supplement day care costs. Parents and families may be eligible for this supplement if a single parent family or both parents in the home are either employed or attending a vocational-orientated education program during the hours when the client is not in school or a program and the family is unable to find an after school program or group daycare that can meet the client’s needs at the same cost of a typically developing child.
All generic (community) resources will be reviewed, including In Home Support Services (IHSS) Protective Supervision, which may serve as a generic resource that meets the same need as daycare.
In the process of determining if the child is eligible for daycare services, families will be required to submit the following forms if applicable:
- Daycare Application
- Employment Verification Form for both parents
- Applicant-Vendor Disclosure Statement DS1891
- Vendor Application & HCBS Provider Agreement
- Daycare Disclosure Information
- Parent’s class schedule
- Client’s school calendar
- Family Cost Participation Program (unless client has full scope MediCal)
- IHSS Notice Of Action
If eligible for daycare services, the parent will be responsible for locating an appropriate daycare provider.
